The supraspinatus muscle in the shoulder is a quite small muscle and it may not be the only muscle that is tightened in the joint.
As with any muscular rigidity, we have to ask ourselves, what is the cause of involuntary tightened muscles in the body? They are induced by a reaction triggered by the autonomic nervous system (ANS), specifically the Sympathetic component of the nervous system.
There is no other mechanism for this condition in the body.

If a chiropractor is saying that tension in the spine muscles is causing occipital neuralgia (neck pain), the message he/she is sending is most likely that your answer may be found outside of their office. It does not seem to be a message that more sessions with a chiropractor will help.
There is nothing wrong with chiropractic therapy, but other than offering short-term relief, it can do little to relax tightened muscles in the spine and neck.

1. Figure out what is causing the pain. All pain has a cause and figuring this out in the first step to dealing with hip and lower back pain.
Assuming you’ve seen a doctor and were told that you don’t have any major issue, such as cancer or infection that is causing the pain, you can probably assume it is simply Tension-Based Pain.
